To change your name on your account:

  • Open our Membership Application form by clicking here.
  • Fill in the following sections:
    • Name (with new name) and Member Number in Section A.
    • Print the completed form and sign with your new name.
    • To validate your new name IAACU requires either a copy of your social security card or Drivers License showing your new name.
  • Fax the completed form and copy of Social Security Card or Drivers License to (309) 557-2419.  You can also mail them to us if you do not have access to a fax. (Find our mailing address here.)

To change ownership of your accounts
(add or remove people from your account):

  • Open our Membership Application form by clicking here.
  • Fill in the following sections:
    • Name and Member Number in Section A.
    • Sections B, C, D, and E with people you are adding. Make sure to select Joint Owner or Beneficiary for each person. If only the primary member is to be associated with the account, leave B, C, D, E blank.
    • Print the completed form and have the primary member and any joint owners sign.
